Updating your Forms

As a childcare facility, our top priority is always the safety and well-being of the children in our care. One important aspect of ensuring this is maintaining accurate and up-to-date enrollment forms for each child.

We ask that parents please take the time to review and update their child’s enrollment form whenever there are any changes to their contact information, emergency contacts, medical needs, or other important details. This allows us to provide each child with the best possible care and support, as we have the most complete and accurate information available.

In addition, it is a licensing requirement for childcare facilities to keep enrollment forms up-to-date. We appreciate your cooperation in helping us stay compliant and maintain our high standards of care.

Thank you for entrusting your child’s care to our facility, and please don’t hesitate to reach out if you have any questions or concerns. We are always here to support you and your family.

Essential tips for updating and completing your forms:

  • Use a separate form for each child.
  • Fill out all sections.
  • Once filled out, print the document in full and bring it to your Center director.
  • Personally discuss all changes with your Center Director.

Feel free to save this document in a safe location on your computer. Throughout the year, you will be able to return to it to make additional changes or updates.

Golden City C.D.C., Inc. (Golden Wings Academy, Inc.) is contracted with all major Resource and Referral (R&R / Subsidy) agencies that provide low-income families free childcare.

New Subsidy Families 

Are you a low income family and are in need of free childcare? Follow the below steps to see if you qualify: 

 1. Identify the Resource and Referral agency in charge of your zip code below. 

2. Contact the agency to find out how you can apply and qualify for childcare assistance. 

 3. Once approved inform the agency you want to bring your child to Golden City Child Development Center in Carson 

4. The Resource and Referral agency will send us your contract with approved hours. For faster processing ask them to fax the contact. 

 5. Contact Golden City Child Development Center at (310) 630-0863 and check to see if there is an opening for your child’s age group and discuses possible start dates. 

6. Once we receive the contact we will contact you to fill out the enrollment application

Golden Wings Academy operates year-round and docs not close for an extended period during Summer or holiday sessions. We understand this causes undue hardship on families, which is why we only close on the days of major holidays.
